Guardianship
A guardian is a person appointed by the court to make decisions on behalf of an individual who has been determined unable to make certain decisions independently. These decisions often relate to healthcare, living arrangements, benefits, and other matters necessary to ensure safety and well-being.
As a court-appointed guardian, I work closely with the individual and their support network to develop a comprehensive, person-centered care plan that addresses both immediate and long-term needs. I include my clients in decision-making to the fullest extent possible and prioritize their preferences, values, and dignity.
My role includes medical advocacy through direct communication with the medical team, advocating for appropriate care and services, and conducting regular visits to ensure living arrangements and supports continue to meet the individual’s needs. Adjustments are made thoughtfully and proactively as circumstances change.
Care Management

Some individuals need assistance coordinating care as they age or experience changes related to disability, illness, or life circumstances. In many cases, family members may live out of state or be unable to provide hands-on support.
Care management begins with a comprehensive assessment to identify areas where assistance is needed. I then work collaboratively with clients and families to develop a person-centered plan that supports safety, daily functioning, and quality of life.
Services may include assistance with benefits, coordination of in-home supports, connecting with service providers for tasks such as housekeeping or meal preparation, and identifying appropriate residential care when needed. Care management is flexible and tailored to each individual situation.
Medical Power of Attorney

Some individuals recognize the need for support and are cognitively capable of appointing a Medical Power of Attorney. As Medical Power of Attorney (MPOA), I work closely with clients to understand and document their wishes regarding medical care should they become unable to make decisions independently.
I assist with capturing these preferences in advance directive documents and ensure they are shared with physicians and other appropriate providers. When acting as MPOA, I advocate for medical decisions that align with the individual’s expressed values and goals.
When appropriate, I may also provide care management support, including coordination of medical appointments, transportation, and communication with providers, to help ensure continuity of care.

*I work in the Denver Metro Area and Front Range, including Jefferson, Denver, Douglas, Adams, Arapahoe, Broomfield, Boulder, Clear Creek, Gilpin, and Larimer Counties.
